PROGRAM APPLICATIONS/ PARTICIPANT FORMS AND DEADLINES
Application Deadline: February 23, 2007 (postmarked)
Notification: no later than
March 31
The notification of your selection along with instructions to complete
various forms, such as visa applications, medical insurance application,
etc, will be sent no later than March 31.
Deadline for participant forms and payment: May 11
Please submit the following participant forms and agreements and a payment
for est. $265 (see cost), payable to "University
of Illinois":
- Pre-departure Curriculum Development Workshop housing form (in pdf)
- Russian visa application form (in pdf) plus 1 passport quality photo with white background and your valid, signed passport (The passport will be returned to you during the pre-departure workshop at U of I). Click here for our visa instructions (in pdf).. PLEASE READ THE INSTRUCTIONS CAREFULLY. Visas will be processed by Perry International.
- Acknowledgement of Risks and Release of Responsibility form (in pdf)
- Data Sheet (in pdf) This information will be used set up the medical/evacuation insurance through the UI Study Abroad Office from CISI. For more information on the CISI insurance, click here.
- Health questionaire form (in pdf) - We do NOT need your doctor's signature. We just need the information.
- US Embassy registration form
