APPLICATIONS AND DEADLINES

Because the Fulbright-Hays GPA grant application for this program is still pending (notification date: late February, 2004), the application for the seminar in Russia will be processed in two phases:

Phase 1 deadline (initial short application): February 15
In Phase 1, the initial short application along with your cv or resume and a list of three references is due by this deadline. Depending on the number of applications that we receive, Phase 2 will be likely limited to those who apply by February 15.

Phase 2 deadline (final completion of application): March 22
Y ou must submit the statement of purpose and letters of references by this deadline.

Notification: April 1
The notification of your selection along with instructions to complete various forms, such as visa applications, medical insurance application, etc, will be sent.

Deadline for participant forms and payment: May 19
Please submit the following participant forms and agreements and a payment for $240 (see cost) by this deadline (payable to University of Illinois).

Program Contract
Russian Visa application form (in pdf) plus 1 passport quality photo with white background and your valid, signed passport (The passport will be returned to you during the pre-departure workshop at U of I) - PLEASE READ THE INSTRUCTIONS CAREFULLY.
Acknowledgement of Risks and Release of Responsibility form
Data Sheet (in pdf) - will be used set up the medical/evacuation insurance and provide contact information to the US Consulate in Russia
Health Questionnaire form (in pdf)- needs to be signed by your doctor/nurse.
